I grant Jay On Top youth football camp the right to photograph and/or video record of my child while football related activities during the Camp. I understand that these visual materials may be used for promotional, educational, or other purposes by the Camp, including but not limited to print, online, and social media platforms.

Youth Foodball Camp Waiver and Release of Liability

I, the undersigned, as the parent or legal guardian of the above-named participant, hereby give permission for my child to participate in the Youth Football Camp organized by Jay On Top Enterprises.

I understand that participation in any sports activity involves a risk of injury, including serious injury, and by allowing my child to participate, I voluntarily accept and assume all risks associated with such participation.

In consideration for allowing my child to participate in the camp, I agree to the following:

    1.    Release of Liability: I, on behalf of myself and my child, hereby release, waive, discharge, and hold harmless Jay On Top Enterprises, its owners, directors, coaches, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child during or in connection with participation in the camp.

    2.    Medical Treatment: I authorize Jay On Top Enterprises staff to obtain medical treatment for my child in case of injury or emergency if I or my listed emergency contact cannot be reached.

    3.    Acknowledgment of Rules and Conduct: I acknowledge that my child is expected to follow all rules and instructions given by camp staff and understand that failure to do so may result in removal from the camp with no refund.

    4.    Insurance Responsibility: I understand that Jay On Top Enterprises does not provide medical insurance and that I am responsible for any and all medical expenses incurred as a result of my child’s participation in this camp.

By signing below, I certify that I have read and understood this waiver and that I voluntarily agree to its terms.
